My filter head measures 21" circumference and 15" tall. trying to get the model# so i can order it.

Does this Clamp Assembly include the O Ring and metal circle to hold the O ring?

Hi Marc, unfortunately, no this lid is only packaged and retailed with the clamp assembly by the manufacturer.
