Do the 3 stainless steel mounting bolts come with the diving board or must I purchase those separately?

Yes, the "board-to-base" mounting bolts are included in the package of a new S.R. Smith Frontier II Diving Board. If you were to need to replace the bolts on a Frontier II, the correct hardware kit would be the 69-209-032. Hopefully this helps!

My diving board is cracked & old (about 30 yrs.). It is mounted on two parallel bars which are shaped like upside down U s not like most of the boards’ pictured with the solid base. It is attached through the back end of the board with two bolts & bolt covers (which are corroded & I don't know how I will remove them) & there are thick rubber bumpers underneath the board which wrap the U's. Can you find me a replacement without tearing up the concrete?

If you are looking to simply replace the board and use the existing stand then we would suggest the S.R. Smith Frontier IV Diving Board. It is specifically made to work with U-Frame diving board stands with 2-Hole Bolt patterns. The Frontier IV's distance between the back bolts and the fulcrum pad in the middle is 40". I would suggest measuring this distance just to be sure everything will line up where it needs to be.
